Interakt Contour zoekt een ambulant begeleider (senior) die zelfstandig bij cliënten thuis werkt en een hoofdverantwoordelijkheid draagt voor het zorgdossier. Je stelt zorgplannen op, bewaakt voortgang en schakelt tijdig bij.
Je functioneert als spil met gemeenten en andere organisaties en ondersteunt cliënten en naasten in complexe situaties. Het werk is betekenisvol, met ruime opleidings- en ontwikkelmogelijkheden, 24–32 uur per week, met uitzicht op vast dienstverband en een salaris volgens
