Meedichtbij zoekt een maatschappelijk werker voor onafhankelijke cliëntondersteuning Wlz en Metgezel. Je ondersteunt bewoners met Wlz-indicaties, onderzoekt hulpvragen en biedt advies terwijl de regie bij de cliënt blijft.
Je combineert langdurige zorgcasussen met ondersteuning en begeleiding, werkt in een team van twaalf collega’s en houdt overzicht tijdens casuïstiekbesprekingen. Thuiswerken is mogelijk en er is ruimte voor opleiding en ontwikkeling.
