Tussenvoorziening zoekt een Ondersteunend Begeleider die principalmente in avonden, nachten en weekenden werkt op diverse locaties in de regio Utrecht en Amersfoort. Je bent aanspreekpunt voor cliënten en collega’s buiten kantooruren.
Je biedt ondersteuning bij dagelijkse vragen, signaleert veranderingen en helpt bij de-escalatie van onrustige situaties, terwijl je structuur en nabijheid bewaart. Daarnaast werk je samen met ketenpartners en teams op locatie.
