District Nursing Sister​/Senior Staff nurse

Overview

The post holder will work as part of a District Nursing Team to provide nursing care without direct supervision for patients registered with GPs within Midlands Partnership NHS Foundation Trust based at Balance st in Uttoxeter.

• Participate in the ongoing assessment, planning, implementation and evaluation of programmes of care adhering to policies and procedures.
• Maintain strict confidentiality regarding all information concerning patients, staff and sensitive Trust business, in accordance with Caldicott Guidance and Data Protection Act 1998.
• Participate in the provision of effective pre and post registration learning experience for students and new members of staff.
• Hold responsibility for a defined caseload of patients drawn from a GP practice population.
• Assume responsibility for directing team coordination.
• Assist the Operational Lead by conducting delegated tasks instrumental in team management and care coordination.
• Lead in organising the induction and clinical supervision of students and new team members.
• Accept responsibility for doing a quality job, aspire to achieve, share a corporate pride in achievements and strive to act as an ambassador for good quality.

• Post is advertised for DN but will consider Senior staff nurse for the right applicant.
• Hold a specialist practice qualification.
• Be skilled in assessment of patient care needs and in the development, implementation and evaluation of care packages.
• Acquire and employ active nurse prescribing skills.
