PRN Speech Therapist (ST) - Home Health - Uvalde

The Home Health - Speech Therapist will be responsible for providing speech therapy services to patients in their homes. The Speech Therapist will work with patients to develop treatment plans, monitor progress, and adjust treatment as needed. The Speech Therapist will also work closely with other healthcare professionals to ensure that patients receive comprehensive care.
Location: Uvalde
Salary: Pay Per Visit including mileage reimbursement.
Responsibilities- Conducts patient assessments and assists the physician in evaluating the level of function by applying diagnostic and prognostic functional ability tests
- Assists in the development of the Plan of Care
- Perform patient visits in a timely, professional, and appropriate manner per standards of care.
- Evaluate and provide skilled speech language therapy to individuals in a home setting, under the care of an attending physician; the home setting may include single or multi-family homes, assisted living, independent living, or memory care.
- Evaluate and treat patients with language impairments, motor speech disorders, dysphagia, cognitive communication difficulties, voice disorders, and those requiring alternative or augmentative communication.
- May work with patients who have experienced a stroke, and those with progressive diseases such as Parkinson’s disease and dementia.
- Treats patients to communicate effectively by expressing thoughts according to the patient's condition. Treatments may include lip and tongue mobility exercises, and audio-visual aids
- Observes, records, and reports to the physician the patient's response to treatment and changes in the patients’ conditions
- Instructs patients in relaxation to optimize benefits of speech therapy intervention
- Instructs the patient, the patients significant others, and other members of the health care team in areas of Speech Therapy in which they can participate
- Visits patient according to Plan of Treatment; completes a progress note for each visit and submits notes in a timely manner
- Participates in staff conferences and committees as necessary
- Attends two in-services yearly
- Provides in-service to agency staff yearly
- Current license to practice Speech Therapy in Texas
- Graduate of an Accredited American Speech and Hearing Association, or its equivalent, program.
- Minimum of 1 year of experience in home health
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Excellent time management and organizational skills
- Current driver's license with current Automobile Insurance.
