About Our Transition to Adulthood Service

Hillingdon'sTransition to Adulthood Service is committed to supporting care-experienced young people to achieve successful, independent and fulfilling adult lives. Wework alongside young people aged 16 to 25, providing practical support,guidance and advocacy to help them navigate key life transitions including housing, education, employment, training, health and emotional wellbeing. We recognise that the transition into adulthood does not end at 21 and so to strengthen our offer, we are developing a dedicated approach for young people aged 21 to 25 that focuses on early intervention, prevention and sustained engagement.

This will enable us to maintain meaningful relationships with young people, proactively identify emerging needs and risks, and re-engage those whomay have become disconnected from services.

Working closely with housing providers, health services, education and employment partners,community organisations and wider council services, the team will coordinate support that is tailored to each young person's individual circumstances andgoals. Our approach is strengths-based, relationship-focused and centred on empowering young people to make informed decisions and maximise their opportunities.

As aPersonal Advisor, you will play a key role in supporting young people toachieve their aspirations and navigate the opportunities and challenges of adulthood.

You will:
  • Work alongside col leagued and partner agencies to assess needs and plan for successful transitions into adulthood.
  • Hold a caseload of care experienced young pepople aged 21+ including young people with complex needs such asmental health challenges, offending histories and immigration matters.
  • Support young people as need arises, including when in crisis.
  • Act as a strong advocate for young people, supporting them to make informed decisions about housing,education, employment, training, health and wellbeing.
  • Provide practical support to helpyoung people develop independence, including assistance with moving home,accessing benefits and establishing a first tenancy.
  • Build effective partnerships with housing providers, health services, education settings, employment services and the voluntary sector.
  • Maintain accurate and timely records, reports and case notes in line with legislative and organisational requirements.
  • Monitor and track outcomes foryoung people, including accommodation, education, employment, training and immigration status.

We are looking for individuals who are passionate about improving the lives ofcare-experienced young people and who share our ambition to help every young person reach their full potential.

You willhave:
  • Experience working with cared for children, care experienced young people or vulnerable young people.
  • A good understanding of the issues facing care-experienced young people as they transition into adulthood.
  • Knowledge of housing, welfare benefits, education, employment and immigration pathways.
  • Strong communication and relationship-building skills, with the ability to engage and motivate young people.
  • Experience of working collaboratively with statutory, voluntary and community organisations.
  • A commitment to safeguarding,promoting welfare and delivering high-quality, person-centred support.
  • Excellent organisational skills andthe ability to manage competing priorities effectively.
  • A reflective and solution-focused approach, with a commitment to continuous professional development.
