Position Summary:

We are looking for a Physician (MD/DO) to lead rounding efforts in assisted living settings throughout the Twin Cities. You’ll develop care plans, engage families, and collaborate with interdisciplinary teams—all in a flexible, mobile, and high-touch practice environment.

• Visit patients on a regular schedule in designated senior living communities
• Diagnose and manage both chronic and acute health conditions
• Support end-of-life planning and advanced directives where appropriate
• Collaborate with on-site teams to improve health outcomes and avoid unnecessary transfers
• Foster meaningful patient-provider relationships

• MN-licensed MD or DO in good standing
• Background in internal medicine, geriatrics, or family medicine
• Passion for person-centered care and team collaboration
• Reliable transportation and valid driver’s license
