Phar Merica is seeking a highly capable and motivated individual to join our team. Phar Merica is a non-retail pharmacy that focuses on providing exceptional customer service and meeting the pharmacy needs for hospitals, rehabilitation facilities, long-term acute care centers, and other specialized care centers nationwide.

Responsibilities
  • Evaluate physician medication orders for appropriateness, drug interactions, and administration routes. Make therapy recommendations as appropriate.
  • Maintain patient database with current and past medical history, clinical data, and medication records.
  • Order and evaluate laboratory tests for monitoring high-risk conditions, discussing results with the healthcare team.
  • Collaborate with healthcare team to provide clinical and technical support.
  • Document all communications with healthcare providers and patients in the medical record.
  • Implement policies for the safe procurement, storage, and disposal of drugs.
  • Maintain documentation in accordance with regulatory agencies and policies.
  • Report medication errors to the Pharmacy Manager.
  • Assist staff with procedures related to medication management.
  • Perform other tasks as assigned.
Qualifications
  • Bachelor’s Degree in Pharmacy;
    Pharm D preferred.
  • One year experience in long-term care.
  • Licenses/Certifications as required by regulations.
  • Detail-oriented, goal-oriented, team player, with strong communication skills.
