Responsibilities- Evaluate physician medication orders for appropriateness, drug interactions, and administration routes. Make therapy recommendations as appropriate.
- Maintain patient database with current and past medical history, clinical data, and medication records.
- Order and evaluate laboratory tests for monitoring high-risk conditions, discussing results with the healthcare team.
- Collaborate with healthcare team to provide clinical and technical support.
- Document all communications with healthcare providers and patients in the medical record.
- Implement policies for the safe procurement, storage, and disposal of drugs.
- Maintain documentation in accordance with regulatory agencies and policies.
- Report medication errors to the Pharmacy Manager.
- Assist staff with procedures related to medication management.
- Perform other tasks as assigned.
- Bachelor’s Degree in Pharmacy;
Pharm D preferred. - One year experience in long-term care.
- Licenses/Certifications as required by regulations.
- Detail-oriented, goal-oriented, team player, with strong communication skills.
