Practical Nurse - Patient Aligned Care Team

The Licensed Practical Nurse is responsible for the delivery of care to Veterans in the primary care clinic. Functions as a team member under the guidance of a Registered Nurse; is accountable for adhering to established policies, protocols, standards of care and practice. The LPN is responsible for the treatment, observation, and care of the Veteran within the scope of practice to assist in the overall PACT clinic.

Duties include but not limited to:

* The LPN/LVN actively assists with the care coordination and communicate needs of high-risk

patients to the team.

* Performs a wide range of clinical and administrative functions on a daily basis including

but not limited to; completing clinical reminders, conducting patient data intakes, answering

secure messages, multi-level patient tracking, schedule preparation (scrubbing), and reinforcing

patient education.

* Assists with diagnostic procedures, such as biopsies, which include preparing the patient,

setting up sterile field and instrumentation, and monitoring the patient's condition when

applicable to the outpatient clinic setting.

* Recognizes, reports, and documents significant, as well as subtle changes in a patient's

physical condition and/or emotional condition or changes to higher level independent provider such

as ARNP, RN, MD, or assigned Supervisor.

* Demonstrates basic in preparation, administration, and documentation of commonly prescribed

oral, topical, subcutaneous, intra-muscular, and other medications as permitted by facility

policies and procedures. Provides and documents patient education of medications.

* Use of Computerized Patient Record System (CPRS), and Bar-Coded Medication

Administration (BCMA), software per policy.

Pay:
Competitive salary and regular salary increases When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).

Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)

Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.

Parental Leave:
After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
