• Coordinate with other social service providers for comprehensive family care
• Participate in team meetings and case reviews
• Assess family needs and develop plans to address them
• Provide case management services such as supervised visits, parent aide, behavioral aide, drug screens, crisis intervention, and advocacy
• Conduct regular home visits to monitor progress
• Offer parenting education and support
• Collaborate with social workers, therapists, and teachers
• Maintain detailed case notes and records
• Provide transportation services

• Bachelor’s Degree in human services (e.g., Social Work, Psychology, Sociology, Counseling, Criminal Justice) OR High School Diploma with 5 years of relevant experience
