Responsibilities- Coordinate with other social service providers for comprehensive family care
- Participate in team meetings and case reviews
- Assess family needs and develop plans to address them
- Provide case management services such as supervised visits, parent aide, behavioral aide, drug screens, crisis intervention, and advocacy
- Conduct regular home visits to monitor progress
- Offer parenting education and support
- Collaborate with social workers, therapists, and teachers
- Maintain detailed case notes and records
- Provide transportation services
- Bachelor’s Degree in human services (e.g., Social Work, Psychology, Sociology, Counseling, Criminal Justice) OR High School Diploma with 5 years of relevant experience
