What you’ll do as a Home Health Speech Therapist/Language Pathologist (ST)

• In partnership with the Case Manager, provide skilled services to assist patients with breathing, speaking, and swallowing functions.
• Evaluate and treat speech, language, cognitive, and fluency disorders that affect communication.
• Serve as an educator, patient advocate, and member of the interdisciplinary care team.

To Be a Success as Speech Therapist/Language Pathologist (ST)

• Current unrestricted CA Speech Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver’s License.
• Knowledge of individual development and human behavior related to illness/injury and cultural influences on health care.
• Knowledge of state and federal home‑care regulatory guidelines preferred.
• Experience in home health care highly preferred.
• Experience with high‑acuity patients is a plus.
• Experience working with an interdisciplinary team highly desired.
• Case Management fundamentals highly desired.
• Self‑directed, empathetic, critical thinker, outstanding interpersonal skills, leadership, organized.
• Prior to scheduling an interview, candidates complete a computer literacy and competency assessment.

Anticipated Wage Range

Speech Therapist: $ – $. Starting pay is commensurate with relevant experience. 21st Century offers generous bonus incentive plans, comprehensive health benefits and a 401(k) with up to 4% match.
