Territory: Solano County (Vallejo, American Canyon, Hercules) and minimal visits to Napa County*
What you’ll do as a Home Health Speech Therapist/Language Pathologist (ST)- In partnership with the Case Manager, provide skilled services to assist patients with breathing, speaking, and swallowing functions.
- Evaluate and treat speech, language, cognitive, and fluency disorders that affect communication.
- Serve as an educator, patient advocate, and member of the interdisciplinary care team.

- Current unrestricted CA Speech Therapist License.
- Current BLS/CPR Certification.
- Valid CA Driver’s License.
- Knowledge of individual development and human behavior related to illness/injury and cultural influences on health care.
- Knowledge of state and federal home‑care regulatory guidelines preferred.
- Experience in home health care highly preferred.
- Experience with high‑acuity patients is a plus.
- Experience working with an interdisciplinary team highly desired.
- Case Management fundamentals highly desired.
- Self‑directed, empathetic, critical thinker, outstanding interpersonal skills, leadership, organized.
- Prior to scheduling an interview, candidates complete a computer literacy and competency assessment.
