Licensed Vocational Nurse / Licensed Practical Nurse (LVN/LPN)

We are seeking a Licensed Vocational Nurse / Licensed Practical Nurse (LVN/LPN) to join our outstanding healthcare team at Solano Post Acute.

• Admitting, transferring, and discharging residents.
• Conduct daily rounds to observe residents’ physical and emotional status.
• Medication administration.
• Monitor resident care provided by unlicensed staff.
• Maintain confidentiality of all resident care information.
• Promote a culture of safety to ensure a healthy practice and living environment.

• LVN/LPN with current licensure to practice in the state.
• Strong communication and organizational skills.
• Basic computer knowledge or willingness to learn.

• Experience in a Skilled Nursing Facility (SNF) environment.
• Direct care nursing experience with multiple residents.
• Behavioral/Memory care experience.
• Experience with Point Click Care  (PCC).
