About the RoleAs a Care Manager you will be responsible for providing comprehensive case management services to clients. In this dynamic role you will work closely with individuals who are transitioning from the justice system as well as individuals experiencing homelessness by helping them navigate various challenges and barriers they may face. This position assists and aids program participants with effectively setting, managing and attaining their goals to become self‑sufficient and financially independent whether through employment or assistance with social services.
The Care Manager may perform home and encampment visits, engage in outreach services with and without community partners, provide transportation for clients, work a flexible schedule, and must be willing to accommodate participants who do not have reliable transportation.
We offer Care Manager positions ranging from Levels I‑III based on experience and to promote professional development. A role as a Care Manager Level II & III requires advanced skills and experience in managing complex cases, high‑level documentation, and providing expert guidance and support to participants.

Vallejo, CA – in office & community; 1 remote day may be granted after 90‑day introductory period

- Carry a caseload of 18‑22 participants maintaining regular contact (at least monthly, based on acuity level) with program participants. Conduct outreach to meet clients where they are including curbside communities, shelters, and other places people may be living
- Conduct initial assessments to identify clients’ needs, strengths, and barriers
- Develop and implement personalized case management plans for each client
- Provide ongoing support, counseling, and advocacy to clients, ensuring they have access to necessary resources and services
- Collaborate with community organizations, social service agencies, and healthcare providers to coordinate and facilitate access to housing, employment, education, healthcare, and other support services
- Monitor clients’ progress and provide regular updates to the program director and other team members
- Maintain accurate and up‑to‑date client records, case notes, and documentation
- Participate in team meetings, case conferences, and training sessions to enhance professional development and share best practices
- Commit yourself to treating each community member with respect and dignity
- Adhere to laws regarding confidentiality and reporting requirements; maintain knowledge of HIPAA certification standards
- Bachelor’s degree in social services, psychology, or a related field, with relevant certification from a four‑year program and/or equivalent experience
- Extensive experience working with low‑income and/or homeless populations, demonstrating advanced clinical skills and case management expertise
