Adoration Home Health and Hospice is seeking a passionate, dedicated Home Health LPN to join our team in Van Buren, AR. You will provide expert care, administer medications, and support patients while maintaining independence and dignity.

• Provide 1:1 care for clients following the plan of care at all times while maintaining dignity and independence
• Administer medications, feedings, oxygen, and ostomy care as needed per client’s plan of care and assessment results
• Accurately document nursing actions, communicate with family, case manager, physicians, and document progress and outcomes for established goals; inform physician, case manager, and families of changes in patient condition and needs
• Coordinate or order supplies, medications, and feedings to ensure adequate amounts are always available
• Ensure all patient needs are met under physician instructions

• LPN/LPT/LVN degree from an accredited nursing college with current unrestricted registration and license in the state of practice
• Valid driver’s license, acceptable driving record, and proof of car insurance
• CPR professional certification
• 1+ year nursing experience preferred
• Home health care experience preferred
