Geriatric Care Physician General

Take on the role of EDiCare Physician at Vancouver General Hospital. This locum, full-time position focuses on managing elderly patients and facilitating their swift return home.

As part of the innovative EDiCare program at Vancouver Coastal Health, you will work collaboratively to ensure frail elderly patients receive exceptional care while minimizing the need for hospital admission. By engaging effectively with families and healthcare providers, you will create comprehensive care plans that align with the philosophy of keeping patients closer to home. Your expertise will be critical in managing complex cases and planning safe discharges.

Key Responsibilities:

• Create and manage safe discharge plans with healthcare teams

• Deliver compassionate, patient-centered care

• Collaborate with the Emergency Department for geriatric cases

• Build rapport with interdisciplinary team professionals

• Communicate effectively with families about care plans

Requirements:

• Valid and full CPSBC License required

• CCFP certification is mandatory

• Passion for geriatrics and home care philosophies

• Experience in interdisciplinary collaboration essential

• Excellent communication skills necessary
