SALARY RANGE: CAD $42.27/Hr – CAD $52.81/Hr
OverviewVancouver Coastal Health (VCH) is hiring a Social Worker to join the Home Health team in Vancouver, BC. The role involves providing clinical and case management services to clients experiencing complex health and social needs.
Responsibilities- Provide case management and clinical social work services to a select population of clients and families.
- Conduct social and psychosocial assessments and deliver individual, family, and group counselling services.
- Maintain client records, attend interdisciplinary clinical/case conferences, and provide Home and Community Care assessments.
- Identify applicable community resources and coordinate care plans to support client health needs.
- Collaborate with multidisciplinary team members and external health care professionals to achieve client‑centred care.
- Master’s Degree in Social Work.
- Two (2) years’ recent experience in case management with geriatric clients or adults with chronic and acute illness.
- Current full registration with the British Columbia College of Social Workers.
- Valid BC Driver’s License and personal vehicle for local travel.
- Comprehensive knowledge of case management principles and practice.
- Experience applying systems theory and evidence‑based practice in assessment and intervention.
- Strong communication, conflict resolution, and counselling skills.
- Ability to work independently, lead projects, and collaborate within interdisciplinary teams.
- Basic computer literacy for client care information systems.
