Social Worker - Case Manager

SALARY RANGE: CAD $42.27/Hr – CAD $52.81/Hr

Vancouver Coastal Health (VCH) is hiring a Social Worker to join the Home Health team in Vancouver, BC. The role involves providing clinical and case management services to clients experiencing complex health and social needs.

• Provide case management and clinical social work services to a select population of clients and families.
• Conduct social and psychosocial assessments and deliver individual, family, and group counselling services.
• Maintain client records, attend interdisciplinary clinical/case conferences, and provide Home and Community Care assessments.
• Identify applicable community resources and coordinate care plans to support client health needs.
• Collaborate with multidisciplinary team members and external health care professionals to achieve client‑centred care.

• Master’s Degree in Social Work.
• Two (2) years’ recent experience in case management with geriatric clients or adults with chronic and acute illness.
• Current full registration with the British Columbia College of Social Workers.
• Valid BC Driver’s License and personal vehicle for local travel.

• Comprehensive knowledge of case management principles and practice.
• Experience applying systems theory and evidence‑based practice in assessment and intervention.
• Strong communication, conflict resolution, and counselling skills.
• Ability to work independently, lead projects, and collaborate within interdisciplinary teams.
• Basic computer literacy for client care information systems.
