POSITION SUMMARY
Reporting to the Health Director and supporting the RN & OT Care Coordinators, the Home Support Supervisor (HSS) oversees and coordinates the day-to-day scheduling of clients that require home support services, ensuring that the Home Makers (HMs) and Registered Care Aides (RCAs) provide quality and culturally safe home care services in community.
KEY RESPONSIBILITIES
A Registered Care Aide by profession, the Home Support Supervisor is responsible for the following key functions:
Providing exemplary front-line home care services
Using a trauma-informed approach, promotes a culturally safe and therapeutic care environment in support of client safety, independence, and well-being
Adhering strictly to the Home Support Service Plan as delegated by the RN & OT Care Coordinators, provides personal care supports by assisting clients with their ADLs & iADLs including dressing, grooming, and bathing, as well as with their mobility
Monitors and reports on client’s home environment as needed to ensure client and staff safety and well-being
Maintains a clean and healthy environment for the client by performing light housekeeping tasks such as cleaning, dusting, sweeping and vacuuming areas of the home used by the client
Reports any changes in client’s condition to the RN & OT Care Coordinators to help ensure timely assessment and intervention
Providing direct supervision to home care staff (HMs & RCAs)
Acts as the main point of contact for the home makers and registered care aides
Assists in orientation and training of new home care staff
Monitors performance of home care staff, providing coaching and mentorship as needed, to ensure the highest standards of culturally safe and quality care is provided at all times
Ensures adherence to TWN policies and procedures
Promotes teamwork, goodwill, and collaboration amongst the home care staff
Provides joint visits with home care staff as needed to help address any reported concerns
Supports staff in reporting any changes in client’s condition to the RN & OT Care Coordinators to help ensure timely assessment and intervention
Provides coverage for HMs & RCAs as needed
Providing administrative & coordination support to the home care team
Oversees and coordinates day-to-day scheduling of home care services in support of delegated care duties as per the Home Support Service Plans
Liaises with external home support agencies as needed to supplement staffing needs as they arise
By liaising with the RN & OT Care Coordinators, triages urgency of home support visits, prioritizing urgent visits over non-urgent ones based on client health care needs
Updates home support service schedules to ensure priority health care needs are met, notifying clients, families, and home care staff of any schedule or staff changes
Supports various administrative duties including but not limited to drafting payment requisitions for home care invoices; supporting staff timecard entries; supporting with quarterly and annual reporting as needed; and documenting minutes for home care team meetings
Provides home care services reporting as required to help inform quality improvement initiatives and to help meet internal and external reporting requirements
Supporting Community Health & Wellness
