LAMP Community Health Worker Role

Join LAMP Community Health Centre as a Community Health Worker focused on equitable care. Engage clients and facilitate programs that enhance health and well-being across diverse communities.

Reporting to the Health Promotion & Community Support Manager, you will conduct client intake, support holistic lifestyle plans, and work collaboratively with community partners. You will play a vital role in creating health promotion strategies that address the social determinants of health, facilitating a nurturing environment for healthy aging.

Key Responsibilities:

• Deliver education and health promotion activities

• Facilitate outreach and community development efforts

• Build effective partnerships with community organizations

• Assist with communication and program awareness

• Oversee data collection and reporting tasks

Requirements:

• Relevant post-secondary education

• At least 3 years in community health or outreach roles

• Strong facilitation and communication abilities

• Understanding of health equity principles

• Familiarity with specific Etobicoke community needs
