Position: Vancouver Medical Coordinator – Supported Housing
Be a driving force for health equity as a Medical Coordinator in Supported Housing with Vancouver Coastal Health. Lead vital services that support unhoused and Indigenous communities.

This role is pivotal in coordinating medical services and ensuring collaboration across various teams such as Mobile Crisis De-escalation and Indigenous Health Outreach. You will advocate for continuity of care and implement quality improvement initiatives within a social medicine framework, significantly impacting community health outcomes.

Key Responsibilities:

• Oversee medical leadership and performance of interdisciplinary teams

• Support the integration of services in outreach and supported housing

• Facilitate quality improvement initiatives across all healthcare teams

• Authorize scheduling and recruitment processes for medical staff

• Manage critical incident reports and improve safety protocols

Requirements:

• Solid background in managing healthcare teams effectively

• Familiarity with homelessness and supported housing issues

• Expertise in equitable and trauma-informed care practices

• Skills in operational management and planning
