Your role will integrate care with community resources effectively.
This role at the Inner City Integrated Care Team requires a dedicated Community Liaison Worker to assist clients facing mental health challenges. You will coordinate efforts among diverse healthcare professionals and provide necessary advocacy for individuals with complex needs. Through planning and facilitating groups, you’ll foster supportive environments that empower clients.
Key Responsibilities:
• Liaise with community health resources
• Facilitate groups for client support
• Monitor and document client well-being
• Assist with daily living tasks for clients
• Organize client appointments and advocacy efforts
Requirements:
• Health Care Worker certificate or equivalent
• Two years' recent experience with mental health clients
• Knowledge of substance misuse and recovery practices
• Ability to operate effectively in diverse communities
• Valid BC Driver's license and access to a vehicle
