Reporting to the Social Work Site Leader, you will bring your Master’s degree in Social Work and at least one year of relevant experience to provide essential services to patients and families. Your expertise will be vital in conducting biopsychosocial assessments, offering consultative support, and aiding in discharge planning processes.
Key Responsibilities:
• Conduct comprehensive biopsychosocial assessments for patients
• Provide consultative support and resources to staff and families
• Lead interdisciplinary discharge planning and clinical conferences
• Develop educational training for staff and interdisciplinary teams
• Maintain accurate records in compliance with industry standards
Requirements:
• Master’s degree in Social Work and registered with BC College
• Minimum one year of related experience in a designated area
• Strong communication skills with all health care providers
• Ability to work collaboratively within teams and handle conflict
• Engaged in research activities and continuous professional development
