Take your clinical pharmacy skills to Downtown Community Health Clinic in Vancouver, BC, as a part-time Clinical Pharmacist. Deliver personalized pharmaceutical care while working in a team-focused setting.
This position involves close collaboration with health professionals from Fraser Health and other partners to ensure optimal medication management for our patients. Responsibilities include patient counseling, developing individualized pharmacotherapy plans, and contributing to research efforts in the pharmacotherapy field.
Key Responsibilities:
• Provide individualized pharmacotherapy and patient counseling
• Collaborate with health care providers on treatment plans
• Lead contributions to case studies and quality initiatives
• Participate in education and mentorship for pharmacy interns
• Review and validate prescriptions for safety issues
Requirements:
• Registered Pharmacist in British Columbia
• Accredited Hospital Pharmacy Residency Program completion
• Knowledge of clinical pharmacy and evaluation processes
• Effective communication skills in patient care settings
• Ability to prioritize workloads and manage documentation
