Work as a Community Liaison Worker with Vancouver Coastal Health at the Lily Lee Community Health Centre in Vancouver, BC. Support clients' mental health and addiction recovery journeys.
In this vital role with VCH, you will engage clients within the Inner City Integrated Care Team. The position focuses on providing comprehensive support services, advocating for clients, and collaborating with interdisciplinary care teams. Your work will directly impact clients' lives by facilitating access to resources and aiding in daily activities, all while fostering a compassionate environment.
Key Responsibilities:
• Monitor and evaluate client progress regularly
• Provide information on community services available
• Co-lead support groups and education sessions
• Assist clients in daily living activities
• Communicate effectively across disciplines and teams
Requirements:
• Certificate from Health Care Worker program
• Minimum of two years’ relevant experience
• Strong understanding of mental health and addiction issues
• Knowledge of community resources in Vancouver
• Valid BC Driver's license necessary
