Occupational Therapist - Home Health

Come work as an Occupational Therapist with Vancouver Coastal Health (VCH) on the Home Health team at Three Bridges Community Health Centre in Vancouver, BC.

Within the context of a patient/client and family-centered framework and continuum-based care model and in accordance with established standards of professional practice and the vision, values and policies of the organization, the Occupational Therapist plans, delivers, evaluates and provides care coordination to address the needs of the clients and families experiencing functional impairment and acute/chronic disease within community, ambulatory and/or institutional settings.

Collaborates with family members or significant others, interdisciplinary team members, educational resources and other healthcare professionals and/or providers to address the client’s functional/physical/occupational needs and social/cognitive/perceptual/environmental needs, to maximize capabilities, community participation and recovery from or adaptation to illness/impairment to achieve excellence in health services and education. Contributes to the organization and delivery of occupational services by participating in program quality assurance initiatives, meetings, research and education.

About Home Health Programs
Home Health programs provide a range of health care and support services to people living in the community who have acute, chronic, palliative or rehabilitative health care needs.

The Client Population
Home Health clients typically have on-going health issues and/or complex personal and/or social issues, requiring care planning and coordination of appropriate resources. Clients include frail elderly, younger clients with disabilities, and adults with chronic illnesses requiring supports to maintain/promote independence. Three Bridges serves the Downtown, West End, Coal Harbour, Yaletown and Fairview Slopes areas.

The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes.

As per Ministry of Health policy, all health care workers working in publicly-funded health care facilities are required to report their past receipt of certain vaccines or history of certain infections. Collecting these records will allow for offering of any missing vaccines, and for appropriate actions to be taken in the event of any future exposure to a communicable disease or during outbreaks.

For all new hires and appointments to Vancouver Coastal Health, you will be asked to provide this information as part of the onboarding process.
