Home Support Scheduler & Care Coordination
Vancouver Coastal Health (VCH) is seeking a Scheduler to coordinate and schedule Community Health Workers (Home Support Workers) in accordance with established policies, client care plans and applicable collective agreements. The role includes outside of work on-call duties.
Reporting to the Home Support Services Manager or Coordinator, you will ensure coverage, adapt to changing needs and support a high standard of care across communities within the region.
As a Home Support Scheduler & Care Coordination, you will play an important part at Vancouver Coastal Health in Vancouver, Metro Vancouver Regional District, Canada.
