The salary range for this position is CAD $33.49/Hr.

Come work as a Client Liaison Worker at Sheway, Pregnancy Outreach Program with Vancouver Coastal Health (VCH) in Vancouver, BC.
Within the context of a client and family-centred care model and in accordance with established vision and values of the organization, works as part of an interdisciplinary team to provide health promotion education, and social support services to high-risk women, infants and/or children in the Vancouver Downtown Eastside. Facilitates engagement in care by using a holistic, trauma informed, harm reduction, culturally and gender appropriate approach to collaborate with the client and the interdisciplinary team to develop and revise a care plan that supports the client to meet their goals.
Ensures continuity of care across the continuum by liaising with interdisciplinary team members, coordinating and implementing clinical interventions, evaluating client outcomes based on client goals for care, coordinating ongoing care and supporting transition. Develops and maintains partnerships with primary care providers as appropriate and in accordance with the service delivery model. Links clients to appropriate internal and external resources and advocates for clients with various community and/or government agencies.
Acts as a resource for the team by providing information based on CLW role. Consults, confers, and collaborates with interdisciplinary team members, physicians, educational resources and other health care professionals to achieve excellence in client-centred education and research. Supports Sheway Drop In and provides outreach services in the Downtown Eastside.
About Sheway:Sheway is a Pregnancy Outreach Program (P.O.P.) located in the Downtown Eastside of Vancouver. The program provides health and social service supports to pregnant women and women with infants under eighteen months who are dealing with drug and alcohol issues. The focus of the program is to help the women have healthy pregnancies and positive early parenting experiences.
QualificationsEducation & Experience
- Diploma in Community Social Service and two (2) years’ recent related experience working with a marginalized client population with complex health and psychosocial issues, or an equivalent combination of education, training and experience.
- Valid BC Driver's license and access to a personal vehicle for business-related purposes.
Knowledge, Skills & Abilities
- Knowledge of the principles and practices of a client and family centered recovery model.
- Knowledge and experience in Trauma Informed Practice, Harm Reduction and Recovery Oriented Care.
- Broad knowledge of mental health and substance use issues and treatment.
- Broad knowledge of other and available community resources.
- Knowledge and experience in providing culturally safe and competent care.
- Demonstrated ability to provide services through outreach.
- Ability to work independently and in cooperation with others.
- Ability to operate related equipment.
- Ability to communicate effectively, both verbally and in writing.
- Ability to organize and prioritize.
- Ability to observe and recognize changes in clients.
- Ability to establish and maintain rapport with clients.
- Ability to analyze and resolve problems.

- Physical ability to perform the duties of the position.
