Nurse Practitioner, Centre Adolescent and Transitional Urology

This role involves working within a healthcare team dedicated to improving care for adolescents and young adults transitioning from pediatric to adult healthcare. The team includes pediatric urology, adult urology, and nurse practitioners with primary care training, fostering a collaborative and holistic approach to patient care. The position offers flexibility in FTE and care delivery methods, including virtual and in-person options.

The Ledcor Centre for Adolescent and Transitional Urology is an innovative initiative aimed at enhancing the transition from pediatric to adult healthcare for patients aged 15-25, primarily managed by pediatric urology and related clinics. The nurse practitioner will serve as the primary contact for these patients, providing assessment, management, and coordination of their healthcare needs.

The NP will co-manage patients, addressing acute concerns, medication management, mental health issues, and facilitating navigation within the adult healthcare system. They will also engage in quality improvement and research initiatives to enhance patient care and advocacy efforts.

• Comprehensive assessment and diagnosis

• Current registration as a Nurse Practitioner with BCCNM
• Experience is preferred but motivated applicants are encouraged to apply

• Full scope of NP duties
• Knowledge of developmental stages, pathophysiology, pharmacology, and evidence-based practice
• Excellent communication and collaboration skills
• Leadership and critical assessment skills
