Qualicare Home Care BC is seeking a compassionate and organized Registered Nurse (RN) or Licensed Practical Nurse (LPN) to support our clients through comprehensive intake assessments and monthly follow-up visits. This role is ideal for an RN who enjoys building relationships, coordinating care, and using clinical judgment to guide care plans in a home care setting.

Your Role

As an RN in this role, you will focus on clinical assessments, care coordination, and monitoring client progress, rather than routine bedside nursing tasks.



Key Responsibilities :
  • Conduct initial client intake assessments in the home or community
  • Complete monthly follow-up visits to reassess client needs and progress
  • Develop, review, and update individualized care plans
  • Collaborate with Care Coordinators and caregiving staff to ensure care aligns with clinical recommendations
  • Provide clinical guidance and support to the care team
  • Maintain accurate, detailed, and timely documentation
  • Identify changes in client condition and recommend appropriate interventions
  • Ensure compliance with infection control standards and professional practice guidelines
What We’re Looking For
  • Licensed Registered Nurse in British Columbia (in good standing)
  • Current CPR certification
  • Strong assessment and care planning skills
  • Excellent communication and interpersonal abilities
  • Comfortable working independently in the community
  • Strong organizational and documentation skills
  • Negative TB test or chest X-ray
  • Valid driver’s license and access to a vehicle
This is a great role for an RN/LPN who prefers a consultative, assessment-focused position with flexibility and meaningful client interaction—without the physical demands of shift-based bedside care.
