VCH Patient Care Coordinator RN

Elevate patient care standards as a Coordinator RN with Vancouver Coastal Health (VCH) in Vancouver, BC at G.F. Strong Rehabilitation Centre. This role demands expertise in nursing and patient care management.

VCH is searching for a Registered Nurse to facilitate effective patient care coordination. In this position, you will lead nursing staff, supervise care delivery, and ensure optimal patient flow. You will also contribute to administrative tasks and promote collaboration among interdisciplinary teams to achieve quality patient care and education.

Key Responsibilities:

• Coordinate patient activities within defined clinical areas

• Improve patient access between units and clinics

• Provide leadership and clinical resources for staff

• Support administrative tasks including scheduling and research

• Promote education and interdisciplinary team collaboration

Requirements:

• Active RN license with BCCNM

• 3 years' related clinical experience required

• 1 year of supervisory experience preferred

• BC Driver's License may be required

• Proficient communication and leadership skills
