In this role, you'll assess and evaluate patient needs and coordinate essential community resources following discharge from acute care. You should have a strong foundation in nursing principles, demonstrated by five years of experience, including work in community health and quality improvement initiatives. Your leadership will facilitate adherence to minimum Nurse to Patient Ratios and promote continuous learning in nursing practice.
Key Responsibilities:
• Conduct assessments for patient transitions to community services
• Connect clients to appropriate resources based on needs
• Advocate for safe discharges by planning clinical discussions
• Maintain comprehensive client records and assessments
• Coach acute and community staff on best practices
Requirements:
• Current Registered Nurse license with BCCNM
• Minimum five years of nursing experience, with three in community health
• Knowledge of evidence-based nursing practice
• Experience in clinical decision support
• Strong communication and critical thinking abilities
