Working as a Case Manager involves collaborating with a diverse team while focusing on client-centered care models. You will assess clients' needs for residential, hospice, and community services, prioritize their applications for access, and devise tailored care plans.
Your role ensures that clients receive the best possible health services in a timely manner while adhering to regulatory guidelines.
Key Responsibilities:
• Manage client assessments for care eligibility
• Foster interdisciplinary relationships for effective placements

• Navigate legal and ethical care considerations
Requirements:
• Current RN registration from BCCNM
• Three years of relevant community health experience
• Provincial Driver’s License for transportation
• Understanding of care standards for geriatrics
• Strong analytical and interpersonal skills
