In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission, Vision and Values of Fraser Health, and working collaboratively and in partnership with the interprofessional care team in the provision of person‑centered and recovery‑oriented care, the Client Care Coordinator supervises and provides work direction to staff, coordinates staffing coverage, provides clinical leadership, guidance and support to team members and acts as a clinical resource for the interprofessional team.

Responsibilities
  • Provides leadership to the interprofessional team by coordinating client care activities, providing information and consultation to designated care providers and students, reviewing client and family needs, troubleshooting, and guiding the management of clients.
  • Supervises staff by scheduling, coordinating and adjusting work activities, establishing priorities, providing performance feedback, determining training requirements, setting team goals and objectives, and participating in staff selection and evaluation.
  • Evaluates client care in accordance with organizational vision by observing staff, reviewing documentation, interviewing patients and families, investigating incident reports, and providing interventions.
  • Participates in the development and maintenance of safety programs, ensuring client and staff safety by identifying concerns and making corrective recommendations.
  • Coordinates quality improvement and risk management activities, evaluates impacts of changes in clinical practice, conducts safety audits, tracks critical incidents, and recommends practice changes.
  • Participates in staff development, conducts needs assessments, introduces new skills, supports autonomous practice, and organizes in‑service sessions and orientations.
  • Oversees quality assurance, reviews client records for completeness and accuracy, and evaluates program effectiveness.
  • Consults with interprofessional team members and external professionals to resolve client/family issues, communicates confidentially, documents relevant information, identifies resources, and recommends solutions.
  • Participates on committees and task groups to resolve issues and advocate program goals; liaises with other programs, agencies, and hospitals to coordinate client care.
  • Promotes research‑based practice through collaboration, staff awareness, and facilitation of research development.
  • Assists the Manager with planning, developing and evaluating client care, provides input on clinical practice and system improvements, and recommends clinical and administrative practices.
  • Provides clinical leadership and consultation to staff and students, ensures compliance with standards, reviews client needs, promotes collaboration, attends meetings, and discusses referral options.
  • Assists with administrative activities and resource utilization, identifies systems for improvement, provides budget input, assists workload decision‑making, and recommends administrative process changes.
  • Provides consultation or screening of clients in special circumstances, identifies program deficiencies or emergencies, recommends actions, and evaluates admission or discharge criteria.
  • Sets measurable personal performance goals, incorporates organizational values, maintains knowledge of clinical and professional development, and collaborates with managers on professional development plans.
