Vancouver Coastal Health is seeking a dedicated Community Health Nurse for its Home Health Team. In this role, you will collaborate with clients, families, and caregivers to provide comprehensive assessments and coordinate care plans. You will play a critical role in managing care transitions and implementing evidence-based practices tailored to client needs.
Key Responsibilities:
• Conduct comprehensive assessments for clients in their homes
• Develop and implement personalized care plans collaboratively
• Provide direct nursing care and health education to clients
• Facilitate access to community health resources and services
• Document assessments and interventions in electronic records
Requirements:
• Registration with the BCCNM required
• One year of clinical experience with complex health conditions
• Valid BC Driver's License for local travel
• Strong communication and assessment skills
• Knowledge of community health nursing practices
Support community health by delivering exceptional nursing care to diverse clients with Vancouver Coastal Health.
