Licensed Practical Nurse, Long-Term Care

• Provide safe and respectful nursing care, adhering to policies, individualized care plans, and priorities.
• Conduct thorough assessments, monitor signs and symptoms, and communicate with the RN/RPN as needed.
• Prepare patients for procedures, transfers, and admissions, coordinating with the interdisciplinary team.
• Administer medications per protocol, monitor effects, and maintain communication with the team.
• Utilize evidence‑based resources to stay updated on nursing practices.
• Respond to incidents such as codes, injuries, spills, and alarms, following procedures.
• Assist with staff orientation, demonstrating procedures and providing support.
• Address patient/resident and family learning needs, provide educational materials, and make referrals.
• Attend care‑planning meetings, provide input, and advocate for patients/residents.
• Collaborate with the team to develop and evaluate individualized care plans.
• Assist with discharge planning based on patient status and external supports.
• Operate and clean equipment, report malfunctions, and handle hazardous materials; perform clerical duties such as answering phones, faxing, and maintaining records.

• Graduation from a recognized program for Practical Nurses or an equivalent combination of education, training and experience.
• Registration with the British Columbia College of Nurses and Midwives as a practicing LPN registrant.
• Current basic cardiac life support certification.
• Knowledge and ability to perform Cardiopulmonary Resuscitation (CPR) procedures appropriate for the area of service.
• Physical ability to carry out the duties of the position.
• Ability to apply decision making and analytical skills.
• Ability to role‑model conflict resolution strategies.
• Ability to prioritize and organize work.
• Ability to teach and identify patient/resident learning needs.
