Licensed Practical Nurse, Long-Term Care

- Provide safe and respectful nursing care, adhering to policies, individualized care plans, and priorities.
- Conduct thorough assessments, monitor signs and symptoms, and communicate with the RN/RPN as needed.
- Prepare patients for procedures, transfers, and admissions, coordinating with the interdisciplinary team.
- Administer medications per protocol, monitor effects, and maintain communication with the team.
- Utilize evidence‑based resources to stay updated on nursing practices.
- Respond to incidents such as codes, injuries, spills, and alarms, following procedures.
- Assist with staff orientation, demonstrating procedures and providing support.
- Address patient/resident and family learning needs, provide educational materials, and make referrals.
- Attend care‑planning meetings, provide input, and advocate for patients/residents.
- Collaborate with the team to develop and evaluate individualized care plans.
- Assist with discharge planning based on patient status and external supports.
- Operate and clean equipment, report malfunctions, and handle hazardous materials; perform clerical duties such as answering phones, faxing, and maintaining records.
- Graduation from a recognized program for Practical Nurses or an equivalent combination of education, training and experience.
- Registration with the British Columbia College of Nurses and Midwives as a practicing LPN registrant.
- Current basic cardiac life support certification.
- Knowledge and ability to perform Cardiopulmonary Resuscitation (CPR) procedures appropriate for the area of service.
- Physical ability to carry out the duties of the position.
- Ability to apply decision making and analytical skills.
- Ability to role‑model conflict resolution strategies.
- Ability to prioritize and organize work.
- Ability to teach and identify patient/resident learning needs.
