This position allows you to utilize your nursing skills to assess and respond to the unique health needs of patients and families grappling with diabetes. Under the supervision of the Patient Care Manager, you will implement evidence-based practices to ensure high-quality nursing care.
Key Responsibilities:
• Assess and prioritize nursing interventions for diabetes patients
• Guide patients in managing their conditions through education
• Record evaluations and necessary adjustments to care plans
• Conduct patient and family learning sessions about diabetes
• Support interdisciplinary teamwork for optimal patient outcomes
Requirements:
• Current RN registration with the British Columbia College
• Diabetes Educator Certification (CDE) preferred
• One year of relevant diabetes education experience
• Strong verbal and written communication skills
• Ability to work collaboratively in a healthcare setting
