As a Nurse Practitioner with VCH, you will assess and diagnose health conditions, initiate treatment plans, and offer guidance to other health professionals. Your expertise will contribute to collaborative care and evidence-based strategies aimed at enhancing the quality of health services for frail seniors in Vancouver's communities.
Key Responsibilities:
• Conduct comprehensive patient assessments and diagnoses
• Initiate treatment and prescribe medications as needed
• Collaborate with multidisciplinary health teams
• Implement evidence-based health improvement strategies
• Participate in quality assurance activities and peer reviews
Requirements:
• Current Nurse Practitioner registration with BCCNM
• Experience in a similar role preferred
• Strong pharmacological and diagnostic skills
• Ability to collaborate with health professionals
• Commitment to ongoing professional development
