Join Rain City Housing as a Nurse Level 3, focusing on mental health and addiction care in Vancouver, BC. This role involves assessments, treatment planning, and collaborative crisis intervention.
The ACT Nurse plays a crucial role in the interdisciplinary team to deliver client-centered care that supports individuals' mental health, addictions, and chronic conditions. With three years of relevant experience, you will utilize your knowledge of the BC Mental Health Act and apply harm reduction principles while consulting with hospitals and coordinating with community services.
Key Responsibilities:
• Assess client physical and mental health needs
• Develop and implement personalized treatment plans
• Provide crisis intervention and support
• Monitor medication administration and wound care
• Maintain communication with health clinics and housing providers
Requirements:
• Bachelor’s Degree in Nursing (RN or RPN)
• Current BCCNM registration
• Three years in mental health/addiction services
• Valid BC class 5 driver’s license
• Ability to perform outreach duties
