Vancouver Coastal Health seeks a Licensed Practical Nurse (LPN) to join the Lily Lee Community Health Centre. This position emphasizes client and family-centered care, alongside collaboration with an inter-professional team. You will engage in assessing, planning, and documenting care for patients dealing with chronic disease and mental health concerns.
Key Responsibilities:
• Assess client needs within mental health contexts
• Develop and implement personalized care plans
• Document and evaluate client care effectively
• Collaborate with nurses and social workers on patient care
• Promote Indigenous Cultural Safety and support diverse populations
Requirements:
• Graduation from a recognized Practical Nurse program
• Current LPN registration with BCCNM
• Valid B.C. Driver’s license
• Experience in mental health and substance use care
• Basic computer literacy for clinical tasks
