Position: ACT - Nurse Level 3 (0.6 FTE)

Position Summary

ACT nurses assess, identify, plan, implement and evaluate the care required to assist ACT team clients in meeting their physical, psychiatric, social and spiritual needs. The Nurse works within the team’s interdisciplinary clinical framework to provide client-centered and strengths-based care in accordance with the BCCNM Standards of Practice and consistent with the mission, vision and values and established policy and protocols of Rain City Housing and the ACT team.

Many of the clients are certified under the Mental Health Act and knowledge of the Act and its implications and limitations is essential to working as an ACT nurse. The Nurse provides treatment services that are both inclusionary and flexible for individuals with mental illness, addictions and/or chronic medical conditions and ensures integration and continuity across the care continuum.

Responsibilities

Assess clients, perform intake, mental and physical health and substance use assessment, develop treatment plans, provide crisis intervention, consult with hospitals for care planning and discharge, perform wound care and medication monitoring and administration, coordinate with community health clinics, hospitals and housing providers, and maintain interdisciplinary team relationships to provide appropriate treatment services based on best practices and client needs.

Qualifications
  • Bachelor’s Degree in Nursing (RN or RPN).
  • Current practicing registration with the BCCNM (BC College of Nurses and Midwives).
  • Three (3) years recent related experience providing treatment services to individuals with mental health and addiction concerns, or equivalent combination of education, training and experience.
  • Valid class 5 BC Driver’s licence with acceptable driver abstract and personal vehicle available for outreach visits.
Knowledge & Abilities
  • Broad knowledge of diagnoses, treatment and support resources in the areas of mental illness and substance use.
  • Knowledge and experience with harm reduction principles.
  • Basic knowledge of legislation and government policies and procedures that relate to client, including the BC Mental Health Act.
  • Demonstrated understanding of oppression and marginalization, specifically as related to people with experience of a mental illness, addiction, or homelessness.
  • Ability to work with individuals from diverse backgrounds in an urban environment and willingness to provide flexible and creative care.
  • Ability to work both independently and collaboratively as a member of a multidisciplinary team.
  • Physical ability to perform the duties of outreach nursing, including light lifting (up to 15 lb) and walking/stairs in community settings.

Additional Information
  • Hourly Range: $41.42 – $55.91 (BCNU Level 3 RN/RPN)
  • Hours: 0.6 FTE, average of 45 hours bi‑weekly
  • Schedule: Rotating 8‑week schedule; shifts 08:30 – 18:30 both weekdays and weekends; 30‑minute unpaid meal break.
  • Work Site: 283 East 11th Ave., Vancouver, BC
  • Start Date: As soon as possible
  • Background Check: British Columbia Vulnerable Sector Criminal Record Check required.
  • Driver’s Requirement: Valid BC class 5 driver’s licence with clear abstract; personal vehicle required.
