In this pivotal role, Nurse Practitioners lead medical assessments for housebound frail patients recently discharged from hospitals. You will initiate treatments, prescribe medications, and guide other health professionals. Flexibility in your work schedule is essential, often including weekends.

Key Responsibilities:

• Assess and diagnose patients' conditions comprehensively

• Initiate treatments, including prescribing medications

• Collaborate with interdisciplinary health teams

• Develop evidence-based strategies to improve health

• Participate in quality improvement activities

Requirements:

• Current registration as a Nurse Practitioner in BC

• Experience as a Nurse Practitioner is an asset

• Knowledge of pharmacology and patient management

• Strong communication and collaborative skills

• Ability to evaluate health research literature
