Position: Community Nurse for Older Adults at VCH
Become an integral part of the Cambie Older Adult Mental Health team at Vancouver Coastal Health as an Older Adult Outreach Nurse (RN/RPN) in Vancouver, BC. You'll support older adults facing mental health challenges, delivering compassionate care in the community.

This role requires a Registered Nurse or Registered Psychiatric Nurse with a strong background in geriatric mental health and addictions. You will conduct psychogeriatric assessments, crisis intervention, develop treatment plans, and offer individual counseling. Collaborate with healthcare teams to facilitate integration and continuity of care for vulnerable older adults.

Key Responsibilities:

• Conduct psychogeriatric assessments and crisis interventions

• Coordinate care and community resources for clients

• Provide counseling and psycho-education to individuals and groups

• Facilitate treatment planning and case coordination

• Consult with families, caregivers, and healthcare professionals

Requirements:

• Active registration with BCCNM as an RN or RPN

• Minimum three years of related experience in geriatric care

• Valid B.C. Driver’s License and access to a vehicle

• Knowledge of psychiatric treatment and gerontology

• Demonstrated ability to develop effective care plans
