Join the Southern Granville Park Lodge as a Long Term Care Practitioner in Vancouver, providing compassionate care for the elderly in a part-time setting. Engage in diverse responsibilities while collaborating with a dedicated team.
This permanent part-time position is created due to an existing practitioner's departure, allowing for an immediate opening to support a 107-bed long-term care home. You will be involved in medication reviews, holistic care, and mentoring staff while maintaining comprehensive documentation. Contribute to care conferences and take part in on-call duties for residents as needed.
Key Responsibilities:
• Deliver interdisciplinary care to long-term care residents
• Conduct medication reviews and participate in on-call duties
• Contribute to ongoing care conferences and events
• Ensure accurate chart documentation and after-hours program involvement
• Mentor new staff and assist in care-home onboarding
Requirements:
• Strong background in long-term care services
• Commitment to holistic, patient-centered strategies
• Effective mentoring and teamwork abilities
• Proficient in using Point Click Care EMR system
