Position: Outreach Nurse for Older Adults at VCH
Vancouver Coastal Health is seeking an Outreach Nurse (RN/RPN) for the Cambie Older Adult Mental Health team. This role focuses on providing compassionate care for older adults with psychiatric disorders, primarily through community outreach and coordination.

As an Outreach Nurse, you will engage deeply with older adults facing challenges like dementia and other psychiatric conditions. Your experience in geriatric mental health is essential for conducting assessments, coordinating care, and facilitating support sessions. This position requires you to work collaboratively with families, healthcare providers, and community resources to ensure the best possible outcomes for our clients.

Key Responsibilities:

• Perform psychogeriatric assessments to evaluate older adults

• Coordinate treatment planning and community resources

• Facilitate individual and group therapy sessions

• Provide direct nursing care as needed

• Consult with caregivers and general practitioners regularly

Requirements:

• Current registration as RN or RPN with BCCNM

• Minimum three years in geriatric mental health

• Valid BC driver's license and access to a vehicle

• Knowledge of concurrent disorders and crisis intervention

• Strong counseling and communication skills
