Position: Registered Nurse for Older Adult Outreach
Join Vancouver Coastal Health's Older Adult Mental Health team and make a difference as an Older Adult Outreach Nurse (RN/RPN) in Vancouver, BC. This crucial role supports older adults facing psychiatric challenges and fosters their recovery within the community.

You will provide specialized care to older individuals experiencing complex medical and psychiatric issues, ensuring high-quality support and integration across care services. Your responsibilities include conducting assessments, crisis interventions, and facilitating recovery planning. Collaborate with various stakeholders to ensure comprehensive care for patients.

Key Responsibilities:

• Carry out psychogeriatric assessments and crisis interventions

• Support case coordination and treatment planning for clients

• Conduct individual and group counseling sessions

• Liaise with families and healthcare providers effectively

• Coordinate community resources and educational programs

Requirements:

• Current registration as an RN or RPN with BCCNM

• Three years of experience in geriatric mental health

• Valid B.C. Driver’s License and access to a vehicle

• Strong knowledge of concurrent disorders and geriatric psychiatry

• Proven ability to foster therapeutic relationships
