Join Vancouver Coastal Health as an Older Adult Outreach Nurse (RN/RPN) on the Cambie team, dedicated to supporting seniors experiencing complex psychiatric conditions. This position offers an opportunity for meaningful engagement and community involvement.
You will play a vital role in assessing and treating older adults facing dementia and psychiatric disorders. With your background in geriatric mental health, you will conduct psychogeriatric assessments and collaborate with families and healthcare teams for integrated care. Delivering education and counseling will be crucial for promoting wellness and connectivity in the client community.
Key Responsibilities:
• Conduct comprehensive assessments for older adults
• Coordinate and implement treatment plans effectively
• Facilitate therapy sessions and educational programs
• Liaise with families and other healthcare providers
• Deliver direct care and support outreach services
Requirements:
• Active RN or RPN license with BCCNM
• Three years experience in mental health and addictions
• Valid BC driver's license and reliable vehicle
• Familiarity with gerontology and psychiatric standards
• Excellent interpersonal and communication abilities
