Sentrex Health Solutions is seeking a Field Nurse Case Manager to join the Patient Services team. This field-based role requires travel to clinics within the assigned region and close collaboration with physicians, nurses and care navigators to support patient programs.
You will enrol patients, provide disease and therapy education, coordinate funding, and ensure timely access to therapies while maintaining strict confidentiality and adherence to program protocols.
