Salary Details The salary range for this position is CAD $49.20/Hr.
- CAD $63.47/Hr.
Job Summary Come work as a Nurse Clinician/CRN with Vancouver Coastal Health (VCH) at Three Bridges Community Health Centre in Vancouver, BC!
Within a comprehensive care coordination model and within the context of a client and family centred care model, working across multiple sites/clinics/services, applies specialized theory, clinical expertise and knowledge relating to designated/specific client populations to promote the delivery of safe, ethical and quality client care. Conducts one-on-one and group education/facilitation/counseling sessions to staff and/or patients/clients/families as needed. Promotes excellence in client centred care, education and research.
Participates in direct client and family care when required. Practices within the context of a client and family centred care model and in accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice for registered nurses or registered psychiatric nurses. Practices within the scope of practice and code of ethics as set by the organization as per VCH policies and guidelines/clinical practice documents and within own competence.

Come join our team that supports Vancouver Community Adult Day Programs, Overnight Respite, and Meal Services. In partnership with contracted service providers, our programs help increasingly complex clients stay at home as long as possible while providing caregivers much needed respite. We collaborate with stakeholders in Vancouver, Richmond, and Coastal to improve service delivery, quality of care, as well as client and caregiver experience.
ABOUT HOME HEALTH PROGRAMS:
Home Health programs provide a range of health care and support services to people living in the community who have acute, chronic, palliative or rehabilitative health care needs.
THE CLIENT POPULATION:
Home Health clients typically have on-going health issues and/or complex personal and/or social issues, requiring care planning and coordination of appropriate resources. Clients include frail elderly, younger clients with disabilities, and adults with chronic illnesses requiring supports to maintain/promote independence. Three Bridges serves the Downtown, West End, Coal Harbour, Yaletown and Fairview Slopes areas.
Qualifications Education & Experience
Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM) is required.
Completion of post basic specialty nursing certificate/program and three (3) years' recent, related experience (Home Health or Palliative Care) that includes experience developing and delivering education programs or an equivalent combination of education, training and experience.
Valid BC Driver's License and access to personal vehicle for local area travel may be required.

Knowledge of relevant clinical practice standards/guidelines and care pathways.
