As a Care Coordinator, you will utilize your nursing skills to provide the best in resident care. Supervise the care team, mentor staff, and conduct thorough assessments, ensuring compliance with Alberta Health Services regulations. Establish care plans tailored to each resident's needs and manage medication administration effectively.

Key Responsibilities:

• Provide high-quality nursing care using the nursing process

• Supervise, mentor, and guide care staff regularly

• Conduct assessments for residents transitioning into the community

• Establish individual priorities and Person Centre Service Plans

• Ensure compliance with regulations through monthly audits

Requirements:

• Licensed Practical Nurse in good standing with CLPNA

• Experience in senior care is an asset

• Strong leadership skills in care settings

• Knowledge of Point-Click-Care is beneficial

• First Aid and CPR certification required
